I acknowledge that this course and its assessments will be delivered exclusively in English. I understand that, due to the technical nature of the content, limited proficiency in English may significantly increase the workload and difficulty of successfully completing the course.

I consent to undertake this course with full knowledge of these conditions. I further acknowledge that I have the option to arrange a registered or certified translator to assist me during the course, at my own expense. I understand that any translator must be approved by HAZLOC Training prior to commencement of the course.

I have a minimum of 3 years experience in general electrical installation or inspection practice, a minimum of 2 years experience in hazardous area electrical installation inspection practice.

I already hold the following three competencies: Units Ex 002, Ex 008 and Ex 009.

I have a degree, diploma or equivalent in a technical subject attained relevant to the application through documents such as college certificates and vocational Certifications, etc.

I confirm that the information provided in this enrollment form is true and accurate to the best of my knowledge. I understand that HAZLOC International Institute of Training Pty Ltd may request additional information to verify the details provided.

I also acknowledge that I am required to inform HAZLOC International Institute of Training Pty Ltd, without delay, of any matters that may affect my capability to continue fulfilling the certification requirements. This includes, but is not limited to, changes in employment status, health conditions, or any other relevant factors that could impact my ability to maintain my certification.

I am aware of and familiar with the requirements for the IECEx Certificate of Personnel Competence and IECEx Ex Facility Orientation Certificate (EFOC). Should my application for certification be accepted, I understand that these requirements shall be fulfilled.

I declare that I will comply with existing requirements for the IECEx Certification of Personnel Competence Scheme, will not misrepresent the scope of certification and agree to pay the expenses in
connection with my application.

I have no current application pending with any other ExCB;

I declare that all information provided with this application is true and correct.

I understand that all examination materials, including but not limited to questions, answers, scenarios, and any related content, are confidential and proprietary to HAZLOC International Institute of Training Pty Ltd. I agree not to disclose, share, reproduce, or discuss any of these materials with any other person or entity, except as authorised by HAZLOC International Institute of Training Pty Ltd.

I commit to participating in the examination process with integrity and honesty. I will not engage in any fraudulent activities, including but not limited to cheating, using unauthorised aids, or assisting others in such activities.

I agree not to disclose any information regarding the specific procedures, formats, or content of the certification process to any unauthorised person or entity.

I understand that any breach of this Non-Disclosure Declaration may result in the immediate termination of my participation in the certification process, invalidation of any certifications received, and potential legal action by HAZLOC International Institute of Training Pty Ltd.

I understand that completing this course does not guarantee I will pass the IECEx examinations. I acknowledge that these exams are complex and challenging, requiring a high level of effort, commitment, and thorough preparation to achieve a passing result.

HAZLOC International Institute of Training Pty Ltd is an Australian company. By accepting this quote or enrolling in any course provided by HAZLOC International Institute of Training Pty Ltd, you consent to any disputes arising from this agreement being governed by and construed in accordance with the laws of Australia. You further agree that any such disputes will be subject to the exclusive jurisdiction of the courts of Australia.
